    How to Generate PDF Reports

    Access reports from your dashboard or the main Reports page in the navigation menu.

    Choose the report you need:

    • • Custom Range: For specific date periods

    Configure Report Parameters

    For Monthly Forms: Select the month and year

    For Cumulative Logs: Choose start and end dates (or "all time")

    For Compliance Reports: Select supervisee (supervisors only) and date range

    Monthly Attestation Process Explained

    Required monthly verification for BACB compliance

    What is a Monthly Attestation?

    The BACB requires both supervisors and supervisees to formally attest (verify) each month that supervision occurred according to requirements. This creates an official record and prevents hour disputes.

    Monthly attestations are legally binding statements confirming compliance. Both parties must review carefully before signing.

    Step-by-Step Attestation Process

    Supervisee Reviews Monthly Hours

    At month end, supervisee verifies all hours are logged and approved. System displays monthly summary with totals and compliance metrics.

    Supervisee Completes Attestation Checklist

    Supervisee confirms the following:

    • ✓ Minimum 2 supervisory contacts occurred this month
    • ✓ At least 1 contact was synchronous (in-person or real-time virtual)
    • ✓ All logged hours represent actual supervision time
    • ✓ Observation with clients occurred at appropriate intervals
    • ✓ All activities comply with BACB fieldwork requirements

    Supervisee Adds Digital Signature

    Supervisee types their full name as a digital signature and clicks "Submit Attestation." System timestamps the signature and notifies supervisor.

    Supervisor Reviews and Verifies

    Supervisor receives notification, reviews monthly summary, and confirms the same attestation items from their perspective.

    Supervisor Adds Digital Signature

    Supervisor types their full name and certification number as digital signature and clicks "Sign & Lock Period."

    Period is Locked

    Once both parties sign, the monthly period is locked. No hours can be added, edited, or deleted for that month. A signed PDF is automatically generated and archived.

    Timing: Complete attestations by the 5th of the following month. For example, January attestation should be signed by February 5th.

    What BACB Requires for Submission

    Required Documentation for BACB Application

    When applying for certification, the BACB requires comprehensive documentation of your fieldwork experience:

    Monthly Experience Forms (Required)

    • One form for each month of supervision
    • Signed by both supervisor and supervisee
    • Shows hours breakdown and compliance attestations
    • Must be submitted in chronological order

    Experience Supervision Form (Required)

    • BACB official form documenting total experience
    • Includes supervisor credentials and contact information
    • Summary of total hours by category
    • Signed original or notarized signature

    Supporting Documentation (Recommended)

    • Cumulative supervision log with session details
    • Compliance summary showing requirement adherence
    • Supervisor's certification verification (copy of cert)
    • Proof of supervisor's 8-hour training completion
    Important: BACB may audit your documentation. Keep all records for at least 7 years after certification. Supervision Hub archives all signed forms automatically.

    Locking Periods After Signing

    Once both supervisor and supervisee sign a monthly attestation, that period becomes permanently locked. This protects the integrity of your documentation and ensures compliance with BACB requirements.

    No one (supervisee or supervisor) can add, edit, or delete hours from locked months. They remain viewable for reference only.

    System creates official signed PDF of monthly form and stores it permanently in both parties' accounts.

    Both Parties Notified

    Email confirmation sent to supervisor and supervisee with copy of signed form and lock notification.

    Archived for Compliance

    Locked periods stored in secure archive accessible for 7+ years per BACB retention requirements.

    If You Find an Error After Locking: Contact your supervisor immediately. In rare cases, system administrators can unlock a period with written justification from both parties. This creates an audit note in your record.
